How can I become an Affiliate?

How are affiliate commissions earned?
Affiliate commissions can be earned in the following ways:
Centerpointe will pay a $30 commission on each sale of Awakening Prologue made to visitors coming through your affiliate link. Your affiliate link can be used on a website, in a newsletter or in an email to direct visitors to our website using either our banners or the link itself.
Campaigns run through PPC services must first lead directly to your website and then to Centerpointe’s through an explicit click on a link or banner on your website. Furthermore, we will not pay any commissions to affiliates bidding for PPC on our trademarks, which include, but are not limited to, the terms Holosync, Centerpointe, Awakening Prologue, among others. Neither may you use a URL, or web site address containing any of our trademarked terms to promote your link. Sales coming directly from affiliate links placed in paid search engine results are not eligible for commission either.
If you do not have a website, and want to participate by “word of mouth” or through a private practice, the only fool-proof way to ensure you are credited for a sale is to have your referrals order through your text based link.
When do I get paid?
Checks are mailed once a month by the 15th of each month. Checks will be made payable in US dollars drawn on a US bank. You will receive a email notification of each sale that your site has generated. It is a good idea to hold onto these notifications for your accounting. We do not send payment via Paypal or wire transfer.
Where can I login to check my account or update my information?
When you sign up, you will receive login details and can access your information any time. If you have questions, please email partners@centerpointe.com.
How do I know if I have had any sales?
Every time you make a sale you will receive an email confirming the sale with the order number and date.
When I click on a link on my web site my affiliate number is doesn’t appear in the address bar after being redirect to the Centerpointe web site. How can I make sure that my affiliate link is still working?
Cookies are used to track commissions on sales, which allows commissions to be correctly paid even if a user does not purchase the item until after many repeat visits. When the customer clicks on your link a cookie is saved for a year on his/her computer. So when they order, the cookie automatically links your affiliate number to the sale which is the only way we can keep track of your sales. You can always check that your link is working properly by clicking your own affiliate link and then checking for the affiliate cookie on your hard drive.
I have referred a lot of customers but they have not used my link to order, do I still get paid for those sales?
Each sale of an Awakening Prologue by phone is eligible for a $30 commission if and only if the customer identifies the affiliate name and number (that is the last 6 digits of your link) at the time of the order. The same is true for orders made via mail and fax. It is your responsibility to notify the customer of this requirement. Under no circumstances will we pay commissions on orders where the affiliate or customer advises us of the referral subsequent to the order. So please make sure that your customers have this information available when they call, mail or fax their order to us. Again, the best way to ensure you are credited for a sale is to have them use either the banner or the text based version your affiliate link.
I have signed up for the affiliate program and have lost my URL ,I need it to put it on my other sites, is there any way I could get you to send it to me?

Do I get a commission on any subsequent sales of Centerpointe products, i.e. deeper levels, retreats, online courses?
Centerpointe only pays commissions on sales of the Awakening Prologue.
